These forms are for producers and anyone who spends or collects money on ACT's behalf. These are provided in both Microsoft Excel format (xls) and Adobe Portable Document Format (pdf). The xls files also work with the free office suite from OpenOffice.org. The treasurer uses both programs and will be happy to help you if you are having trouble or want to figure out how to do something fancy.

Even if you don't want to be reimbursed, or only partly, it's helpful for us to understand the full cost of our productions and to properly credit you for your donation, so keep your receipts and submit your reimbursement requests.

[pdf] For anyone who collects money on ACT's behalf. A printout of this form should accompany any payments given to the treasurer. If you don't want to deal with the spreadsheet (xls), print out the pdf and fill it out by hand.
